Pretty much confirmed that it was Duffy in his Pre Carlisle press conference by saying that it was nothing to do with anything in the dressing room but players arguing back whilst on the pitch when they've just made a mistake... We've all seen Duffy doing that and he's made enough mistakes so it was clearly him.
