That's very true-but it was the government's under Thatcher and Blair particularly that began the outsourcing of NHS services to the private sector, including care homes. Yes, there was certainly empire building and poor productivity in the NHS beforehand but it all got considerably worse with the creation of Trusts. I firmly believe that much of the blame for the terrible state of the service today lies in that decision where numbers of management have grown whilst numbers of front line staff has shrunk drastically. Pay for nurses remains comparatively low but conditions are worse still. Never mind though, I'm sure people standing outside clapping them made up for it.
