My simplistic opinion onnit is-allegedly--

1 Noisy number of people on street outside gate on Leppings Lane at 2-50pm, some with tickets, some not.
2 Bobbies on street radio in to report situation getting worse and need help
3 Inexperienced Senior Officer responds to demands with instruction to let everybody in by opening gate but crucially fails to block off packed central pen through tunnel beforehand to direct fans to side pens where there was room.
4 Afterwards Chief Officers have been cleared of a cover up and that may or may not have occurred pending possible appeal
