An elderly patient was admitted with obvious symptoms of a urinary infection. She was about to be discharged with no urinary infection found. Competent people were surprised and luckily for the patient asked the right questions. The hapless consultant said a test for urinary infection was on his list and "must have been performed" although he expressed surprise that the results were not recorded. The ward sister was asked and she flew into a tantrum saying tests were "always" conducted. Finally, when no record of such test was found, a test was made and lo and behold, severe urinary infection was found. Without this intervention the very frail and elderly patient would have been sent home and very probaly have died. The NHS is not fit for purpose.
